Scholarship Candidates
The following four statements are to be completed by the nominator. (Limit 300 words for each answer.)

4. Describe why the nominee is a deserving candidate for a scholarship.

Grant Candidates
The following five items are to be completed by the nominee. (Limit 300 words for each answer.)
1. Which professional areas do you feel you most need to develop?
2. Why do you feel this conference will be beneficial to your professional growth?
3. Describe your college's budget restrictions and how they will make it difficult for you to attend this conference.
4. List NCMPR national and district conferences or one-day workshops you have attended in the past three years. (If you, personally, have not attended, list the conferences or workshops attended by others in your college.)
5. Provide the name, title and phone number of an individual in your college's business office who can be contacted to confirm your college's restricted travel and budget policies.
